Family Physicians of the Year – the Reg L. Perkin Award

The 2025 Family Physicians of the Year Award recipients

The Family Physicians of the Year Awards—named in honour of Dr. Reg L. Perkin, Executive Director of the College of Family Physicians of Canada (CFPC) from 1985 to 1996—recognize outstanding CFPC family physician members who exemplify what it means to be a family doctor: providing exceptional care to patients and contributing to the health and well-being of their communities. These awards are based on peer nominations, with all CFPC members welcome to submit nominations for their fellow family physicians who are eligible.

The award will be presented during Family Medicine Forum (FMF). The value of the award is up to $2,000 for provincial recipients and up to $5,000 for the territorial recipient, to support travel to FMF. Additionally, the CFPC will cover the cost of a four-day registration for FMF and provide two tickets to the Awards Gala.

Eligibility criteria:

Each nominee must be a CFPC member who has been in good standing for at least one year and is a skilled clinician.

Candidates may be recognized for providing exceptional care to their patients and making meaningful contributions to the health and well-being of their communities.

Candidates may be recognized for their dedication as researchers and educators of future generations of family doctors.

Each nominee must be community based and act as a resource to a practice population.

Candidates must recognize the central importance of the doctor-patient relationship.

Nominees are not eligible if they have received this award in the past three years.

Nomination process:

All CFPC members are invited to submit nominations for the Family Physicians of the Year Awards. Health care colleagues, community leaders, and patients are encouraged to submit letters of support for these nominations. Each of the CFPC’s provincial Chapters will select a recipient and the CFPC will select one recipient for the territories.

For the territories:

Nominations for Family Physician of the Year for the Territories can only be submitted directly to the CFPC National Office. Nominations must be received by 4:00 p.m. (ET) March 9, 2026.

Please contact the provincial Chapter of your nominee for further details and to submit your nomination.

A complete nomination will include responses to the following:

  1. How does the nominee demonstrate outstanding leadership in areas of advocacy, community service, and/or education?
  2. How does the nominee support excellence in family medicine, including community medical services and contributions beyond their medical practice to the community at large?
  3. How does the nominee exemplify the Four Principles of Family Medicine:
    1. The family physician is a skilled clinician
    2. Family medicine is a community-based discipline
    3. The family physician is a resource to a defined practice population
    4. The patient-physician relationship is central to the role of the family physician
  4. How does the nominee demonstrate that they deserve the Family Physician of the Year Award honour?

Although not mandatory, the following elements are strongly encouraged for a nomination package (maximum 10 documents):

  • A recent resume
  • Patient testimonials
  • Additional letters of support from peers, community organizations or groups, health professionals/administrators, etc.
  • Relevant media reports, pictures, etc.
